Telangana Trans Woman Completes Postgraduate Degree in Emergency Medicine

Koyyala Ruth John Paul, 29, a transgender woman from Khammam district in Telangana, has completed her postgraduate degree in emergency medicine. Results announced on Saturday confirmed she passed the MD examination at the Employees’ State Insurance Corporation Medical College and Hospital in Hyderabad. She has taken up a senior resident post at the ESI Hospital in Sanathnagar.
Ruth came out as transgender at age 20 while in the third year of her MBBS course. Her family rejected her. She lost her father in childhood and grew up in modest circumstances. She completed her undergraduate degree at a private college in the city and later worked as a medical officer. Multiple hospitals had earlier declined to employ her because of her gender identity.
Admission to the postgraduate programme required a legal intervention. No dedicated transgender category existed for medical PG seats. Ruth approached the Telangana High Court and obtained a favourable order that allowed her to compete. She was initially considered under the female category but continued to press for recognition of her identity. The court’s direction enabled her to secure the emergency medicine seat.
She described the journey as difficult. Colleagues initially doubted whether she would be accepted, but within months staff began asking for “Ruth madam.” She now earns approximately ₹1 lakh a month in the contractual senior resident position. She plans to apply for an assistant professor post when vacancies are released. She has encouraged other members of the transgender community to pursue medicine and has shared case files with them.
Ruth said many transgender people end up in begging or sex work. She was determined to stand on her own feet and to serve patients. Her family is now supportive because she can earn a regular income. She sustained herself on low stipends during training. The completion of the MD and the residency appointment mark the first time a transgender person has finished a postgraduate medical degree in India under these circumstances, according to her account and local reporting.
The absence of a reserved transgender category for NEET-PG seats forced the court route. The NALSA judgment of 2014 recognised transgender persons as a third gender and directed reservation in education and employment, but implementation in medical admissions has been uneven. Ruth’s case illustrates both the legal avenue that exists and the practical obstacles that remain.
She has already worked in HIV/AIDS care as a medical officer at Osmania General Hospital alongside another transgender doctor, Prachi Rathod. Both secured government posts after earlier private-sector rejections. The senior residency provides clinical experience in emergency medicine and a platform for further academic progression.
Local health authorities and the medical college have not issued a formal statement beyond the examination results. Ruth’s appointment is contractual. She expects to compete for a permanent faculty position when the government notifies vacancies. Her experience is being cited by community members as evidence that postgraduate medical training is accessible once institutional barriers are addressed through the courts.
The achievement adds a concrete data point to the slow expansion of transgender representation in Indian medicine. Two transgender doctors now hold government medical posts in Telangana, and at least one additional candidate is pursuing similar opportunities in Andhra Pradesh.
