Health authorities in eastern Siberia have placed 189 people under medical observation after an employee of an anti-plague research institute in Irkutsk died, local media reported. Alexei Tsydenov, head of the neighbouring Republic of Buryatia, said the woman had died of an unspecified form of plague. Russia's public health watchdog, Rospotrebnadzor, said on Sunday that she had been diagnosed with pneumonia of unknown etiology, and that expanded testing had not detected microorganisms linked to her work.
Those two accounts do not match. A regional leader used the word plague. The federal watchdog used a pneumonia label and said the organisms associated with the institute's work were not found. Both can be true only if Tsydenov was speaking ahead of the lab result, or if the two offices are using different case definitions. Neither has published the test list.

Rospotrebnadzor said the sanitary and epidemiological situation in the Irkutsk region, and in the cities of Irkutsk and Shelekhov, remained stable. Local reports said the employee fell ill after a work trip. Tsydenov tied the death to an outbreak reported near the Mongolian border, in an area linked to Buryatia. Media descriptions put the dead employee at 27. Tsydenov referred to a woman and did not confirm the age in the accounts carried abroad. CNBC reported the observation number as 189, inside a wider phrase of almost 200.
An anti-plague institute is a specific kind of laboratory. Russia inherited a network of these stations from the Soviet system, built to watch natural foci of Yersinia pestis in rodents across Siberia, the Caucasus and the steppe bordering Mongolia. Staff handle samples from those foci. A death in that workforce is why 189 contacts are being watched, even if the federal result says the work-related organisms were not found. Observation is the conservative step. It does not require a confirmed plague death to be ordered.
Why the wording split matters
Plague is treatable with antibiotics if it is caught early, and fatal if pneumonic disease moves through a closed group before treatment. That is the reason a single death at a plague institute produces a contact list measured in the hundreds. It is also the reason a federal statement that testing did not find the organism is the line hospitals will use. If Rospotrebnadzor's result holds, the observation can be stepped down once incubation windows close. If Tsydenov's account holds, the institute's last field trip becomes the exposure event, and the Mongolian-border focus he mentioned becomes the place to sample next.
Until one of those accounts is withdrawn, the public fact is the contact list. One hundred and eighty-nine people are being watched in and around Irkutsk. The dead woman worked at the anti-plague institute. The federal laboratory says it did not find the organisms tied to that work. The leader of the neighbouring republic says the cause was plague. The situation report says the region is stable. Those four sentences are the whole of what has been confirmed.
Observation is the step that does not wait
Contacts of a suspected pneumonic case are watched because the window is short. Antibiotics work early. They do not restore a patient who has already developed the fast form of the disease. That is why a laboratory death produces a list of 189 even while the federal result is still being worded. Shelekhov and Irkutsk city are the places Rospotrebnadzor named as stable. Stable, in a watchdog statement, means no further confirmed cases in the public note. It does not mean the contact list has been closed.
The institute's job is the other fact. Anti-plague stations exist to sample rodent foci, including along the Mongolian border Tsydenov referred to. A staff death after a work trip is the scenario those stations are built to contain. Containing it requires the test result the two offices have not yet aligned. Until they do, the number that can be checked is 189.
